Introduction: Primary renal lymphoma is a rare neoplasm, representing less than 1% of injuries in this organ and their existence has been questionable in the absence of lymphoid tissue in the kidney. It occurs in adults with mean age of 60 years, similar to renal cell carcinoma and the differential diagnosis by imaging is often difficult. Case: We report the case of a female 78 years old whose primary renal lymphoma was diagnosed subsequent to radical nephrectomy and confirmed by histopathology and further treatment. Conclusion: Primary renal lymphoma is a very rare disease whose differential diagnosis of other renal masses is often difficult and sometimes takes place after radical nephrectomy. It requires aggressive treatment with chemotherapy and rituximab, despite of this the prognosis is grim and most patients die within two years of diagnosis. [ABSTRACT FROM AUTHOR]
