Abstract (English):
Introduction. Oral Anticoagulation Therapy (OAC) is the mainstay therapeutic treatment in thromboembolic disorders and, to date, its prescription regards roughly 1.2 million people in the Italian context. Diverse evidence in the literature show how the disease knowledge plays a pivotal role in reaching good level of adherence, as well as in reducing clinical complications. However, specifically in the Italian context, no evidence of validity are available on tools aimed to measure the OAC's knowledge. Therefore, the primary aim of this study is to presents the main results coming from a recent publication on Italian Anticoagulation Knowledge Tool (IAKT) validity, and discuss the state of art, gaps and future prospective on OAC management. Methods. The I-AKT validation process follow systematic and methodological phases: cultural-linguistic validation; content and face validity; construct validity; reliability and stability analysis. Particularly, the construct validity has been tested through a cross sectional and observational study, including two main Anticoagulation Clinics in North Italy, aimed to test tool's psychometric proprieties. Results. The first phases of validity process have been completed with good evidence of agreement between the health care professionals involved in the assessment of item translation and cultural adaptation, its pertinence and relevance on the tool's theoretical constructs. Indeed, 334 participants has been involved for construct validity assessment, confirming the I-AKT mono-dimensionality (i.e., OAC Knowledge) and its ability in discriminating between different knowledge levels. This structure explained the 56.42% of total variance through an Explorative Structural Equation Modelling. Finally, the scale also show both good internal consistency (Cronbach's α = 0.896) and test-retest reliability (r = 0.855; p < 0.001). Conclusion. I-AKT implementation will allow to assess the OAC knowledge in reliable way, ensuring to keep the real health education patients' need and promoting the implementation of personalized care models.
