Abstract (English):
Introduction Studies in the area of childcare show that heavy nursing workloads and unfavourable working environment increase the risk of hospital readmission and worse the quality of care. The aim of the study was to describe current nursing care and the quality and safety of paediatric care in Italy. Methods A cross-sectional observational study was conducted. Data have been collected at three levels: administrative, nurses, caregivers or adult patients. Thirteen hospitals, affiliated to the Italian Paediatric Hospitals Association, with their respective units of ordinary hospitalization were enrolled. Analyses were aggregated by the following clinical areas: medical, surgical, and critical. Results One hundred and sixty-nine operating units were included and 2769 nurses enrolled. The average national staffing differed for the three areas: surgical area 5.93 (DS ±2.67); medical area 5.7 (DS ±3.33); critical area 3.55 (DS ±2.68). The response rate was 80%, 39% and 68%, respectively. 80% of paediatrc nurses reported not optimal levels of quality of care, while patient safety was not perceived as being good by 49% of nurses. Throughout the country, 27% of nurses scored in the high burnout range. The skill-mix ranged from 76% to 82%. Conclusions The results showed weaknesses in staffing, quality and safety of care, highlighting the need to steer policy choices in terms of human resource needs towards the quality and safety of paediatric hospital care. The results of the first Italian and European study conducted on the impact of paediatric nursing care on the quality and safety of care will arise the national and international debate, aimed at improving the patients' clinical outcomes.
