Infectious endocarditis is a rare but still fatal disease. A case of a frequently ill child without underlying cardiopathy,with proven fungal endocarditis at 3 months of age is presented. The diagnosis was made on the basis of the following criteria - positive blood culture (Candida parapsilosis) and the presence of massive mitral valve vegetation, with mitral regurgitation (EchoCG), manifestations of cerebral thromboembolism, anemic syndrome and markers of inflammation. Conservative drug therapy was performed, but due to progressive heart failure and mitral regurgitation, uncontrollable fungemia, surgical treatment was undertaken with extirpation of vegetations from the mitral valve. After 4 months, a second operation was performed due to recurrence of fungal endocarditis with multiple vegetations of MV. A mechanical prosthesis of MV was placed. The young age, morphologically and functionally normal heart, difficult to interpret neurological finding complicate the diagnostic search. The appearance of a cardiac murmur and the echocardiographig finding, positivity of the blood culture, as well as paraclinical data for inflammatory activity point to the diagnosis. Рatients with mycotic endocarditis should be closely monitored due to the possibility of recurrence. In severe cases surgical treatment is indicated together with drug therapy. [ABSTRACT FROM AUTHOR]
