We assessed the efficiency and limitations of using a 10-point self-check sheet on the day of participation in an athletic event to prevent medical accidents in the Kakitsubata Marathon. In 2003, 633 completed the self-check sheet, 17 (2.7%)canceled participation in the race and 617 participated and completed the race. In 2004, 758 completed the self-check sheet, 37 (4.8%)canceled participation in the race, 725 participated and 724 completed the race. The one person that did not complete the race was a 57-year old male, who collapsed with heat stroke and had to receive first aid treatment. The number of people who enjoy sports is increasing, but many accidents happen during such activities. This self-assessment is useful as a screening tool to confirm the physical condition of the participants from the administration perspective in order to provide medical support, and as an opportunity to check one's own physical condition on the day of participation in a marathon race from the participants' perspective.
